Question on USCG Form 719k

I am in the process of upgrading my license to C/M unlimited, and I just finished filling out and completing my USCG form 719k. On the top right corner it says it expires June 30, 2012 and I will be at sea through that date, so is it possible to just send in that form by itself without my other forms in order to get a letter to test? If so, do I just send that form to West Virginia to have them put it in my file or do I have to send it with everything else? Thanks for taking the time to respond.

I believe the date you are talking about is the internal office date for the NMC. I guess they re-evaluate their own form at that time. Fill it out and send it in. See what happens, if nothing else it will stay in your file.
