I found a collection center on the DOT Collection Site locator in the PDF listed, above.
I made an appointment and went there to talk to them about the other requirements:
- Can perform DOT 5 Panel Test that ONLY tests for Marijuana, Cocaine, Opiates,
Phencyclidine, and Amphetamines - The laboratory the specimen will be sent to is accredited by the Substance Abuse
and Mental Health Services Administration (SAMHSA) - The results will be signed by a Certified Medical Review Officer (MRO)
Ok… They said they only do a 10 panel federal test 
They had NO idea about their testing lab, but she looked it up on google and apparently it is accredited by SAMHSA.
They had NO idea who signs what at the lab.
Further, the drug testing form I’d printed out… she said they don’t do anything with that. But it looks like there are spots for the lab and MRO to fill things out. Is that form to be sent with the sample? Or just for me to fill out when I get the info??
Has anyone been through this before? Why am I having such trouble figuring out how this all works??
Any help appreciated–I will call the MMC place on Monday to try to get some answers–I’ll report back here with what I find out (and don’t find out).
But I thought I’d ask here first, since a 2 day wait until I can call.
