I’m looking for a version of CG_719B.pdf that isn’t locked. I would like to be able to type in the information as opposed to hand writing it in a printed form. Any help appreciated - I’m hopin’ to turn in my package today.

Thanks for the link - I was getting pretty frazzled getting all of my “stuff” together. The really good news is that I turned in my package today! YAHOO! The folks at the REC in Seattle were great.
