A fascinating study of what (if the committee is right) had to be near the top of the Black Swan category of failures, the collapse of the giant Arecibo Observatory radio telescope:

Forgot to mention that. Registration required, and is free.

In other words, “free” means you are the product they are selling to someone else. The story is the bait.
Er, it’s the National Academy, chartered by Congress under the Lincoln administration. I’ve been a member of 13 studies and a reviewer of dozens more. Pretty safe.

You can download as a guest. I just did.
Ah, then it is the rare exception to the rule. Thanks for the note.
a fascinating subject. I did read the design life was 10 years. (wtf?) this must of been dependent on standardized maintenance programs? inaction after the first cable pullouts doomed it. it should of been lowered to the ground and kept there till someone coughed up money to fix it. Mostly what I saw in the report are excuses. The CE probably should of risked his job and shut the facility down. It’d be better than being on the payroll when it collapsed! anyway, instead of heads rolling we read a lot about how it was suppose to work, or should of worked. too bad, such facilities are much sought after for research.
