Thanks, GCaptain for highlighting the CSB Accident Investigation Report.

I like this part the best: “Although there have been regulatory improvements since the accident, the effective management of safety critical elements has yet to be established,” Investigator MacKenzie said. “This results in potential safety gaps in U.S. offshore operations and leaves open the possibility of another similar catastrophic accident.”

But, I bet they had every form, checklist and log entry filled out and filed letter-perfect.

Funny that the American Petroleum Institute respectfully disagrees with the findings. I wonder how much they just increased the budget for the lobbyists in Washington to make sure Congress disagrees as well.

They say the first kick was undetected…how can this be with all the instrumentation they have in a driller’s cabin? Asleep at the switch has always been my theory! Why they have not mandated data and voice recorders for drilling shacks after this has always shocked me. Dead men cannot speak to what they witnessed and what actions they took but a recording of everything would be very easy to create.

I also wished the video had more on the actions investigators believe were taken on the drill floor leading up to the explosion. Perhaps it is in the report though and I will be reading it soon.

Most are boobs and all are overpaid. Overpaid like all of us rig hands. I am also surprised there is no voice recorder and/or video recorder in the Drill House…or Bridge, or Engine Control Room. The date recorders are there recording Vessel Management Systems and Drill Floor Zone Management Systems. But as we all know there were a series of factors leading up to this horrible incident.

The well blew out before it blew up…this gave most onboard time to evacuate.
the ones that died were most likely on the rig floor on shift…trying to prevent the
disaster. When the gasses got to the generators,they oversped, blowing electrical
shit which likely lit the bomb.
it was not a pretty sight.

The secondary system on the bop was bad…they continued.
the cement job was bad…they continued.

I didnt read the investigation,but i can promise that if it had not been
bp’s fault, bp would have never forked out all that money…before the
investigation.

Actually now we do have what is called E-Hawk, which allows shoreside and NOV to monitor the BOP system and status. We also do now install driller black boxes on the drill floor, similar to a VDR, also compiles the data from the drillers chair.

Maybe there is a right time to displace the liquid mud with seawater ?

Hopefully the driller black box is someplace else besides on the floor.
