Nvic 04 17

Hello!
I’m applying for OSV CE with III/I STCW https://www.dco.uscg.mil/Portals/9/NMC/pdfs/checklists/mcp_fm_nmc5_238_web.pdf
at the bottom of the checklist it asks for a NVIC 04 17 assessment. I have 07 14 and 17 14 but, 04 17 does not seem to exist. Anybody know what the USCG wants?
