How did it “go wrong”? Sounds as though it succeeded beyond expectations.
IIRC in 1916 in Massachusetts industry over one hundred people were killed “skylarking” (though hand and eye injuries made up the overwhelming bulk of the ~100,000 lost-time accidents).
