What is needed to cross over from Mate AGT to a Mate AGT/Unl. near coastal.Possible?

A friend has 5000T Mate of uninspected fishing vessel and Mate AGT Inland. We are trying to figure out what is needed to get a 3rd mate ticket. He has Oceans. Confusing reading. Thanks for your help
46 CFR 10.403 diagram shows “Mate GL/INL AGT” crossing to “3rd Mate Ocean/NC” after taking an “exam for Mate NC”, whatever that is.
