Would anyone have this form in .doc format that I could use? I have lots of small craft sail/power days I need to account for. Thanks in advance!

You cannot edit that form without purchasing Acrobat Editor. I’m looking for it converted to a .doc format

The resulting doc looks quite usabe to me.
You can print and fill out by hand then scan with a phone app that converts it to a PDF. Just another method.
