I am over arguing the semantics of how AC power works. I have posted enough circuit diagrams for you to see how to connect it if it is something you work on and the USA is not the only place you will find this.

The company had to get custom made UPS for the 220v computers as they would trip when they see power in the neutral as most trying to run 3 pin plugs, no ground.
Very little understanding of this even by the locals.
Pretty scary when there are metal cases around equipment.
At the end of the day whatever you want it call it the usa doesnt have one conductor to ground with 220v
