NTSB report about the collision between the John S, McCain and the Alnic MC. There is a few new details here about the Alnic MC but not really anything that changes the overall story.

This incident is in some ways similar to Eastern Air Lines Flight 401 which crashed in the Everglades while the entire cockpit crew was preoccupied with the burnt out landing gear indicator light.
In the case of the McCain the problem originated with crew error not equipment failure
