I’m in the process of putting together a lockout tagout program for my vessel per our SMS requirements. Though I’m very familiar with carrying out lockout/tagout and the accompanying work, I’ve always used very comprehensive company forms that were written by someone else and part of a whole loto procedure manual. The problem with my task is that I have no guidelines to follow other than to make it happen. Does anyone have a good set of procedures and forms that they wouldn’t mind sharing to use as a guideline? I’m still combing the Web to find something that I may be able to adapt. Thanks in advance for any input.
Try this search “lockout tagout procedure template filetype:doc”
That should pull up something decent, then cross check it to make sure it meets applicable OSHA and SMS requirements you need to meet.

Well, we were going to write it. I wanted a quality document/product to effectively cover all of the bases but, it now seems that the shoreside people are handling it. They have finally decided to make it a standard, company-wide procedure and permit system. Novel idea…I was beside myself that this wasn’t already the case. Can’t wait to see what the finished product looks like… Thank you all for your assistance.
