Nope, they were evidently reported through the NASA ASRS system which anonymizes information.
Aviation is not my thing but I occasionally check this guy out because the way he discusses these things appeals to me. If my former QHSE folks had put out discussions such as this I think it would have been more useful than all the IADC, MSA, etc, etc safety alerts and tables of statistics that they flood a ship’s crew with.
This second one is not about the 737 but really hard to ignore that “resource management” issues have not been overcome in any industry.
