My hope is that you will apply a proven format (AAR, Taproot, etc.) to your investigation. This will be a key event in our time & the sequence of events on what occurred and what will happen after this, will impact us all. We only get one chance for getting the facts right. My recommendation would be to get independent industry subject matter experts involved to verify information received & captured. Learnings from this event can prevent re-occurance.
Key data / information will be the real time well data (indicating well events / parameters). Communications between crews during handover (crew change) and job procedures utilized.
Please learn from the Cullen report, North sea, Piper Alpha, UK sector & focus on the potential impact and future of the industry (enable improvements & prevent creation of restrictions /barriers). A positive outcome would be to make this transparent, clear to the public and with respect towards key stakeholders / persons involved.
I like to thank all that have contributed to this forum as they have kept this simple, factual (with some good corrections) and informative on the sequence of events. This is what most persons have missed from the key stake holders involved for reasons I can understand. Correct & timely communication is key during events like this.
I’m deeply affected with this event & my thoughts are with the missing, family and colleagues. I’m still carrying hope.
